Free practical advice, support and health assessments for all ages. Services we offer are: identification of clients at risk of illness or injury, client assessment and ongoing health monitoring, development of individual care plans with Whānau involvement, health promotion and education, follow up on hospital discharges and GP services, and collaboration with other health providers.
